NJSOPHE, INC. CONTINUING EDUCATION APPLICATION
INFORMATION Thank you for your interest in obtaining BEFORE APPLYING FOR CREDITS PLEASE NOTE: The conference planning committee must have someone who is ches and a member in good standing of njsophe as well experience in health education and planning of continuing education events. Also, please be sure to read the application instructions before designing the conference brochure/announcement. The national commission for health education credentialing (nchec) requires that the conference brochure/announcement contain specific wording regarding continuing education credits for ches. The application for a Standard Event is available through this web site. The instructions for completing the application are attached. There is an application fee of $50.00 and is made payable to NJSOPHE. Please return the application, with five copies and fee to me at least three weeks prior to the event. Once the application is received and materials are reviewed, you will be notified of the number of credits approved. At that time, you will also be supplied with all materials needed for CHES to receive their credits.
