To obtain the NJ SOPHE Member Award Information, the Awards Nomination Form, the Health Educator of the Year form and the Criteria for Program Excellence Form, download and print the following documents, fill out the applicable forms, and send to Jennifer Morgan Kidd. Health Educator of the Year Criteria

The purpose of this award is to recognize a dedicated member of NJ SOPHE, Inc. who has shown an active commitment to Health Education and the organization.

Eligibility

A candidate must be a NJ SOPHE, Inc. member in good standing.

 

Recognition

The nominee selected for the award will receive a plaque, which will be presented during a meeting sponsored or co-sponsored by NJ SOPHE, Inc.

NJ SOPHE, Inc. will notify relevant media of the winner’s achievement.

 

Criteria and Application Requirements

Nominee must be actively involved in NJ SOPHE.

Nominee must demonstrate a commitment to excellence in the practice of health education.

A brief summary of the key accomplishments and distinguished characteristics of the individual must be outlined in the application.

Three signatures of endorsement are required.

The Louise Chut Award for Program Excellence Criteria


The purpose of this award is to recognize outstanding contributions made by a program or project (not an agency) to the practice of health education. The winner will be selected from all qualified entries submitted to the NJ SOPHE, Inc. Awards Committee.

Eligibility

All programs or projects in which a NJ SOPHE, Inc. member is an active participant are eligible.

 

Recognition

Award winners will receive a plaque and be recognized during a fall meeting sponsored or co-sponsored by NJ SOPHE, Inc.

NJ SOPHE, Inc. will notify local officials and relevant local media of the winner's achievements and honors.

 

Criteria

The NJSOPHE, Inc. Award for Project Excellence will be presented annually, but only when the Awards Committee receives a meritorious nomination.

The program/project entries will be judged according to the following:

     

  • The degree to which the program addresses key issues in public health.
  • A health education project that adheres to sound health education principles regarding needs assessment, program development, implementation, and evaluation.
  • The program must have achieved measurable results.
  • The program cited must be one that clearly demonstrated collaborative efforts.
  • The program must have the potential for replication by other agencies.
  • The project staff should have demonstrated commitment to excellence in the practice of health education.

 

Application Requirements

To apply for the award, please provide the following:

     

  1. Completed ‘Nomination & Application Form’
  2. A program summary- no more than three pages in length, typed, double-spaced (specifications on reverse)

 

Program Summary Specifications

The program summary is not to exceed three, typed and double-spaced pages. It should include a discussion of the items described on the Nomination & Application Form, using subtitles provided and in the order listed. Please do not send supplemental documents and attachments.
